*Check to make sure the clinic you are attending has the vaccine you need. Ages 5-11 may receive pediatric-dose Pfizer vaccine, all Coloradans 12 and up are eligible to receive the Pfizer vaccine, and those 18 and up are eligible for the Moderna or Johnson & Johnson vaccine.
